On Saturday, Oct. 6, the Agape Health Clinic will have three physicians seeing uninsured, low income people of Pratt County at the Allied Health and Nursing Building on the Pratt Community College campus, beginning at 8:45 p. m. Doors open at 8 a.m. Patients are seen on a first-come, first-served basis.
Proof of income is required: three months pay stubs, 2011 income 1040 form, copies of any state or federal checks, or, if money is direct deposited, a copy of your bank statement.
Without proof of income, patients will be seen, but no tests will be ordered and proof will be required before retuning to the clinic again. Identification is required also.
The next dental clinic will be held on Nov. 10, for extractions only. Those who would like to see the dentist must be checked by a physician at a regular clinic.
